CIT Y 0 F MAR Y S V ILL E Marysville,Washington RESOLUTION NO./5J;t A RESOLUTION OF THE CITY OF MARYSVILLE ADDING AN ADDENDUM TO THE ACCIDENT PREVENTION PROGRAM BY ADOPTING A PERSONAL PROTECTIVE EQUIPMENT POLICY WHEREAS,the Washington State Department of Labor and Industries has required that the city implement a written accident prevention program;and WHEREAS,the Washington State Department of Labor and Industries also requires that the Accident Prevention Program be customized for each department;and WHEREAS,.the city Safety Committee has reviewed and approved the proposed addendum,a copy is attached hereto;and WHEREAS,the Marysville city Council finds that the adoption of the Personal Protective Equipment Policy is in the best interests of the,City and its employees; BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF MARYSVILLE, WASHINGTON,AS FOLLOWS: 1.The City of Marysville hereby adopts the Personal Protective Equipment policy which is attached here to as EXHIBIT A. 2.The Personal Protective Equipment policy shall apply to all city of Marysville Employees and shall be administered by the City Administrator and/or his designee. 3.The Personal Protective Equipment policy shall be added to the Accident Prevention Program and made a part of the Personnel Rules for the City of Marysville and shall be incorporated therein. ~PASSED by the city Council and APPROVED by the Mayor this 20/-"day of c~1992. I.General: CITY OF MARYSVILLE Personal Protective Equipment Policy RESOLUTION NO.1522 EXHIBIT A The purpose of this policy is to describe and explain when the use of personal protective equipment shall be used. (a)The intent is to protect the employee and control the dangers of injury while working in hazardous conditions and to meet the requirements of the Washington Industrial Safety and Health Act. (b)This policy affects all employees. II.Maintenance: Protective equipment,including personal protective equipment for eyes,face,head,and extremities,protective clothing, barriers,protective shields,and respiratory devices shall be cleaned regularly and generally maintained in a sanitary, reliable working condition. III.Equipment and Usage: (a)Hard Hats:Shall be worn performing construction, repair,or inspection work in or on: 1.Streets,roadways,median strips or adjacent to vehicular traffic. 2.Construction sites where cranes,backhoes,overhead scaffolding or the like are in use. 3.Trenches and other hazardous confined spaces. 4.Or wherever the supervisor,department director,or riSk manager determines that a hazard exists from falling or flying objects. 5.Hair netting and other such protection shall be worn by an employee who has hair long enough to present a hazard while working around machinery. *Note:When hard hat use is for visibility purposes or when no overhead danger is apparent the hard hat may be substituted by an approved orange,yellow or white cap,which one per year will be provided. (Colors will be determined by department supervisor) All employees must have individual hard hats onsite and readily available at all times. (b)Traffic Vest or Coveralls: Employees working within or exposed to vehicular traffic accprev2.doc -September 91 - dkw -Page 1 ~'~,. or made of highly visible or reflectorized material. *Special Note:All flaggers shall have a flagging issued by the proper authority.All flaggers will hard hat,reflectorized vest and carry a whistle. emergency situations Police and Fire personnel are when involved for traffic control.(If possible a reflectorized vest or coat should be used.) (c)Eye and Face Protection: card wear a During exempt Eye and Face protectors shall be provided and worn where machines or operations present the hazard of flying objects,glare,liquids,injurious radiation or a combination of these hazards. *Note:Prior to handling chlorine refer to Emergency Manual for Chlorine Handling,Confined Space Entry,and Respiratory Operating Operations. (d)Respiratory Protection: Respirators shall be provided by the City of Marysville when such equipment is necessary to protect the health of the employee.The employee shall use the provided respiratory protection in accordance with the instructions and training received. Immediate supervisor or lead oerson shall fill out proper paperwork and perform toxic gas checks before entering. *Note:Prior to handling chlorine refer to Emergency Manual for Chlorine Handling,Confined Space Entry,and Respiratory Operating Operations.It is necessary to use the gas detector before entering confined spaces. (e)Full Body Harness and Lifelines: Where workers are employed above the floor,water surface,or the ground and it is impractical to provide temporary floors,staging,ladders,or scaffolds,safety belts,lifelines or lifenets shall be provided and used by employees. No employee shall enter a sewer,sewer flue,storm sewer duct,waterline,tunnel or other similar places without first notifying his supervisor of his intention to do so. He shall wear a full body harness with a lifeline attached,when conditions reguire it. *Note:Prior to handling chlorine refer to Emergency Manual for Chlorine Handling,Confined Space Entry,and Respiratory Operating Operations. The line shall be held by a fellow worker stationed at the opening which he enters.In such cases,signals shall be agreed upon and responded to immediately. accprev2.doc -September 91 - dkw -Page 2 .. Employees working over or on water,where a danger of drowning exists,shall wear a u.s.Coast Guard approved lifesaving device. (g)Shirts or Protective Clothing: Shirts or a reasonable sUbstitute shall be worn by all employees.No employee at any time shall work without some type of protective clothing above the waist. (h)Safety Shoes and Boots: When provided by the city of Marysville shall be worn at all times. (i)Safety Chaps Shall be worn by operator anytime a chain saw is in use. (j)Hearing Protection Shall be worn when the noise level reaches: 85 dba - 8 Hours 95 dba - 2 Hours 105 dba -1/2 Hour over 105 dba - a (See Attached Exhibit "A"for Noise Survey of Marysville equipment.As new equipment is purchased or replaced it may be added to the Noise Survey list.) IV.Responsibility: It shall be the responsibility of the crew supervisor to check that the required personal protective equipment is available to and used by the employee and all safety precautions are in use,on all job sites according to the procedures set forth in this policy. V.Disciplinary Actions: The City of Marysville is concerned about the safety of each employee on the job site and his fellow crew members. Therefore each employee shall be made aware of this policy and supplied with the necessary protective equipment by their department supervisor. Any supervisor not enforcing,or an employee not meeting the conditions of this policy,could face the following: First Offense -Verbal warning and Supervisor record. Second Offense -Counseling session -Supervisor record.
